# UK Medical Device Clinical Evaluation: MHRA Requirements and Post-Brexit Changes

Clinical evaluation is the systematic process of gathering and appraising clinical data to verify that a medical device achieves its intended purpose and has an acceptable benefit-risk profile. For the UK market, clinical evaluation must satisfy MHRA under the UK MDR 2002 framework — which diverges from EU MDR in key respects including the continuing validity of the equivalence approach and MHRA's specific guidance documents.

## Clinical evaluation stages

01

### Stage 1: Identify applicable regulations and device classification

Confirm the device is a medical device under UK MDR 2002 (as amended). Determine device class (Class I, IIa, IIb, III) — class drives the depth of clinical evidence required and whether UK Approved Body review is needed.

02

### Stage 2: Define intended purpose and clinical claims

Document the intended purpose precisely — clinical evaluation must cover all intended uses, indications, patient populations, and body parts. Unsubstantiated clinical claims are a primary MHRA finding.

03

### Stage 3: Literature review

Systematic literature search for clinical data on the device (or equivalent/similar devices). Literature must be searched in recognised databases (MEDLINE/PubMed, EMBASE, Cochrane) with a documented search strategy and selection criteria.

04

### Stage 4: Clinical data appraisal

Evaluate the clinical relevance, scientific validity, and quality of each data source. Weight of evidence assessment — randomised controlled trials have higher weight than case series.

05

### Stage 5: Clinical evaluation report (CER)

Compile a clinical evaluation report documenting the evaluation process, data reviewed, conclusions on safety and performance, benefit-risk analysis, and residual risks. The CER must be signed by a clinically qualified and technically competent evaluator.

06

### Stage 6: Post-market clinical follow-up (PMCF)

Proactive plan to collect clinical data after market launch to confirm safety and performance and identify emerging risks. PMCF plan and reports are required for Class IIa, IIb, and III devices.

Equivalence approachEquivalence to a predicate device remains a legitimate clinical evaluation approach under UK MDR, subject to demonstrating sufficient equivalence in clinical, technical, and biological characteristicsEU MDR significantly tightened equivalence — contract access to equivalent device's technical documentation often required

## Frequently asked questions

### Does the UK accept clinical data generated from EU MDR clinical investigations?

Yes. Clinical data from studies conducted in EU member states that are appropriately designed and reported can be included in UK clinical evaluations. The clinical data itself (patient outcomes, safety data) is not jurisdiction-specific — it is the regulatory framework that differs. EU-conducted clinical investigations under EU MDR are generally acceptable data inputs for UK MHRA clinical evaluation reviews, provided the studies are methodologically sound.

### How does MHRA assess clinical evaluation reports during UK Approved Body review?

For Class IIb and III devices, the UK Approved Body reviews the clinical evaluation report as part of the conformity assessment. The Approved Body examines: whether the systematic literature search is methodologically adequate; whether the clinical data substantiates the claimed intended purpose; whether the benefit-risk conclusion is credible and supported by data; and whether the PMCF plan is proportionate to the residual risks. MHRA may also conduct direct scientific advisory or review functions for certain device categories.
